We hear all the time suggestions for improvement. Or suggestions for outright abandoment of establshed Processes and some time even establsihed Principles. Both listening to and most important making any changes, do a Root Cause Analysis (RCA) of the problem. Most descriptions of a problem are actually descriptions of a Symtpom, not the Cause.
