Have as many layers as fits within your PMO methodology. That’s the minimum (and the approach advocated by PRINCE2). A survey by the Centre for Complexity and Change at The Open University found that a third of project managers are involved in the decision making process but are not the sole decision maker themselves*.

The updated PRINCE2 guidelines that were released this year also include information on tailoring for your environment. It does take a while for practice to catch up with guidance, so expect to see some changes in your PMO soon; we can expect more flexibility with how to apply project management best practice.
